Financial Policies

The Responsible Party is assigned to the Parent/Legal Guardian who is present with the minor child on the initial visit and accepts responsibility for payment of our professional fees. 

The Responsible Party assigned to the account is responsible for payment independent of what a divorce decree or custody arrangement may state.  Reimbursement must be made between the divorced parents.  We will not intervene.  Please make arrangement in advance if someone other than the Responsible Party will accompany the child to the appointment. 

The full payment of treatment is due at the time services are rendered.  For your convenience, we accept cash, check, debit cards, and most major credit cards.  Additionally, we offer Care Credit as a financing option.  The business office will be happy to assist you with an application.   

As a courtesy to you, we will file your dental insurance claims to any company that will pay us directly.  FILING YOUR INSURANCE IS NOT A GUARANTEE OF PAYMENT.  Please understand that your insurance benefits are a contract between you and your employer/insurance company.  Upon each visit to the office, YOU are responsible for payment.  This includes any deductibles, co-payments, balances, and services not covered by the insurance.  

We are happy to file both primary and secondary insurances if you are able to provide the insured’s personal information, complete insurance information/card with correct mailing address and phone number to verify benefits.  If we are unable to confirm eligibility, you may be given the option to reschedule or make a payment in full at the time treatment is provided.    

At treatment visits, we collect a percentage of the total cost of treatment, determined by an ESTIMATION of what your insurance will cover, plus any deductible required by your insurance. In the event of under-payment, we will send you a statement by mail.  Any over-payment will result in a credit on the account.  

It is the responsibility of the Parent/Legal Guardian to know and understand their insurance benefits.  At your request, we are able to submit a pre-treatment estimate to your insurance for a more accurate estimate prior to treatment. 

Our doctors recommend treatment based on your child’s individual needs, not on what insurance will pay.  However, we will do everything possible to maximize your benefit. 

The practice cannot carry balances over 90 days, regardless of insurance.  Delinquent accounts may be charged a 1.5% per month or 18% per year finance charge.  If it becomes necessary for your account to be sent to an outside collection agency, you will be responsible for all costs of collections, including court costs and attorney fees.
